Please find below an entry form for the 2011 Greater Manchester Mini Games.

The Entry Form includes Photo Consent information and form to be completed.

Please return pages 1 and 2 of the form to Lydia Smith at GreaterSport by Friday 24th June.

Page 3 of the form must be brought with the adult in charge of teams to the Mini Games on Friday 8th July.

Teams are welcome to return all forms on 24th June if they have all details completed.
