The role of Person Centred Community Approach within the Local Pilot

The Person Centred Community Approach (PCCA), has featured heavily within the Local Pilot, as it highlights the importance of putting the person at the centre of our services. Previously there might have been a risk that people were referred around the system, rather than taking the time to really understand what the issue is, to ensure that someone gets the support they need to continue with a better quality of life. This also, reduces the pressure on our services, in particular the healthcare system. 

This booklet, produced by the Greater Manchester Health and Social Care Partnership, provides an understanding to what this means, and how it does, and could potentially, work. This is something that each locality within the local pilot is striving do.
