On this page can be found all the documents needed for the monitoring of Sportivate. 

PLEASE NOTE THAT THE CONFIRMATION OF ACTIVITY FORM, REGISTRATION FORMS AND PROJECT REVIEW FORM HAVE ALL BEEN UPDATED FOR APRIL 2012 AND OLD VERSIONS WILL NOT BE ACCEPTED FOR PHASE 3 AND 4 REPORTING.

Before activities start, each SPAA lead must confirm the details requested in the Confirmation of Activity form to feed into a Greater Manchester Sportivate calendar and confirm that activities have started.

Funding for Sportivate projects will be paid on receipt of the attendance register, a completed and signed registration form for each participant and the project review form.

Each participant must complete a Sportivate registration form. There are two versions – one for 14 & 15 year olds, which is to be signed by a parent/carer, and one for 16 to 25 year olds, which the participant signs. Where delivery partners have additional information they need to collect, we recommend merging their form with the Sportivate one. However, please note registration forms will not be accepted unless they include all the information – including the specific data protection wording – shown on the Sportivate forms.

In addition, attendance at sessions must be recorded via an attendance register. A register template is available to download below – if you use your own register, please ensure it records all the same details as the template version. There is a space for you to add your organisation name in the use of data section at the bottom of the page (see section in red)

The data protection participant handout explains exactly how the data collected will be used. This is to be handed out or to be made available at sessions for participants to read. There is a space for you to add the name of the location and deliverers’ data protection contact before distribution (see brackets in red).

Additionally, the data protection information for deliverers document explains the legal obligations concerning the collection and use of personal data provided by people taking part in activities.

Every deliverer must complete a short project review form at the end of the 6 or 8 weeks of Sportivate sessions. This will enable GreaterSport to develop case studies to show the impact of Sportivate on individuals and communities and share good practice County-wide and nationally.
